In order to process your application, we require the following: Government Issued Photo Identification or Birth Certificate for all members of the family, and Social Security card for all members of household.

You may either mail copies to P.O. Box 1547, Mt Vernon, IL 62864, email to ts@hajc.org, or drop off documents at 109 Shiloh Dr. Mt. Vernon, IL 62864.

- In order to apply for a elderly/disabled unit the head of household must meet the definition of Elder or Disabled family as defined by HUD AND have qualifying income. Proof of disability for members under the age of 62 must be provided to the Housing Authority.
